Who We Are

AdvantagePoint Health Alliance – Great Lakes in partnership with UP Health System Marquette/Bell/Portage, Upper Greats Lakes Family Health Center provides free Community Health Worker services to residents of Marquette and Houghton Counties. Our Community Health Workers are trained in communication, interpersonal and relationship building, promoting the health of formerly incarcerated people, understanding trauma, home visiting, conflict resolution, cultural humility, client-centered counseling, mental health first aid and more.

What We Do

Our Community Health Workers can work with individuals to: • Talk about health goals clients may have for themselves and help them develop a plan to move towards those goals • Address any needs they may have that could be affecting their lives, their health and how they access health care, things like transportation, insurance, establishing a relationship with a primary care provider, scheduling appointments, getting prescriptions refilled and connecting patients with community resources to address unmet Social Determinants of Health needs • Provide encouragement, support, and informal counseling/coaching • Learn new skills, increase confidence, and enhance their ability to better manage their own health • Provide health education and information in a way that is easily understood • Communicate your priorities effectively to your provider
